2022-23 OBLBC Membership Application/Renewal

Use this online form for renewing all types of membership in the Oak Bay Lawn Bowling Club. Every member must submit his/her own copy of the form.

You can maintain your own membership record; on line, for example for changes in address.

This form does not "time out", so there is no need to hurry; your input will stay in place until you submit the form.

All fields marked with an asterisk * must be answered, or the form will not submit.

Select from the choices below whether you are renewing or applying for:

A FULL membership, which entitles you to engage in all club activities and vote at club business meetings. You must be considered a full member to play croquet. This includes members under the "family" fee structure.

A FAMILY RATE, Up to four people of one household will receive a first-year full membership for a grand total of $150. This special rate is only available to those who have never belonged to a lawn bowling club.

A DUPLICATE membership, Must be a member of another club and as such have already paid the Bowls BC fee. Classed as a Full Member. May join other Oak Bay members in closed BSI tournaments.

An ASSOCIATE membership, Must be a member of another club and as such have already paid the Bowls BC fee. Play in all summer lawn bowling from when the greens open to when they are closed, at which time Associate Player membership expires. Given a locker and access to the Members Only section of the website. May not play with Oak Bay members in closed BSI tournaments, as they would be representing their home club.

A SHORT MAT AND SOCIAL membership, which entitles you to access to all short mat activities and all social activities all year. Access granted to lockers and to Members Only section of the website.

A SOCIAL membership, which entitles you access to all social activities all year, but not lawn bowling nor short mat bowling.

An UNDER 26 membership, Access to all club activities.

Answer both questions below; type of membership and renewal/new.

The club needs to maintain a register of all members for statutory and administrative purposes. Please supply the membership information listed below.

The club reports certain member information to Bowls BC and Bowls Canada for administrative and statistical purposes. Please answer the questions below. All Bowls BC questions must be answered before the form will submit. (Six questions for full members; two questions for social members.)

The operations of the club are completely dependent on its volunteers. We realize that the circumstances of some members preclude taking on these activities, and some of you have contributed more of your share in the past and are entitled to time off now. But if you are able, don't just shift the burden to others; please do your part and volunteer for at least one activity.

This section contains optional questions that are intended to increase interaction among members and support for the club.

I consent to the following three uses of my contact information. (Select your choices from the dropdown lists. The club's preferred response is at the top of each list.)

There are nine fee categories for renewal for the period April 1, 2022 to March 31, 2023:

1. 1. Full member adult – $250. Fee drops to $125 for new members only after July 15.
2. 2. Duplicate – $215
3. 3. Associate Player – $125
4. 4. Social – $75
5. 5. Junior (age 18 and under) – $35
6. 6. Social and Short Mat member (all ages) – $125
7. 7. Under 26 (between 19 and 25)– $90
8. 8. Life Membership – $250
9. 9. Family Fee - $150 (four family members max). If any of the family members have ever been a member of another lawn bowling club, you are not eligible for this special one year rate category.

   Pick the category that applies to you and the amount of your payment from the Fee Amount choices below.

The option of online payments will be made available to any club member who uses online banking at a Canadian financial institution.

You have the option of paying your fees either:

1. 1. By cheque; or
2. 2. Online, via Interac e-Transfer. Send to obfunds@shaw.ca

As soon as you select your method of payment from the dropdown box below, some advice will be displayed for that method.

You need only make one payment per household (although every household member must submit a separate entry form.) If you are paying for more than one person, please identify all you are paying for in the Comments section at the end of this form.

Immediately after completing this form, please complete the online banking payment or make out the cheque to the club.

The Membership Secretary and the Treasurer have spent a lot of time in the past pursuing members who have simply forgotten to pay, even though they have filled out their renewal forms. Hence the pressure to pay at the same time as you complete the renewal form.

If an individual registers and pays the Club fees and is not able to begin using the membership due to a medical issue( supported by a doctor's written verification ), the member has the option of requesting a full refund of the fees. If a member has begun partaking in any Club activities, no full or partial refund will be made.

Before submitting this form, type your name in the agreement box below to signify that you agree that:

1. 1. The information you have supplied on this form is complete and correct, including the consents to the use of your personal information.
2. 2. Your membership will not become effective until:
   1. (a) the Membership Secretary accepts your application; and
   2. (b) the requisite fee has been received by the club.

RENEWING MEMBERS

If you are updating your existing membership record, you should now see "YOUR UPDATE IS A SUCCESS" at the top of your screen. If not, you have some missing data. Scroll down the form and look for areas highlighted in pink. Fill in the missing data and Update again.

Once you see the YOUR UPDATE IS A SUCCESS message you are given the option to RETURN TO START PAGE. This is in case you want to make more changes to your membership record. If not, you are done.

You will not receive a confirmation record because you yourself can look at what is on file for you at any time, by clicking on the link in the renewal notice that was sent to you.

NEW MEMBERS ONLY

If you are submitting a new application and do not receive an immediate confirmation message after you submit the form "Thank you. Your membership request has been received..." (in blue lettering) , then you have NOT successfully submitted your 2022/23 membership application/renewal.

If there is no confirmation message, scroll back to the top of the form; you will see you have an error message, "There was a problem with your submission . . ." . Check to see which fields on the form you have not completed. (Incomplete fields are highlighted in pink.) Fill in the missing data, then click on SUBMIT again.

In addition to the immediate "Thank you . . ." confirmation message, within minutes you will receive an e-mail showing your complete membership information as submitted. (if you don't receive an e-mail, check your Junk Mail folder; your service provider may have directed it there.)

Please check your input, as shown in the e-mail, to make sure that it is accurate. If you need to make changes, log on to the online form again. Almost all of the previous information from your latest entry will appear; you need only make the changes that you want, then resubmit.
